Family Medicine

Maurice A. Guzman M.D. is originally from the city of Chicago.  Dr. Guzman completed his undergraduate studies at Loyola University of Chicago.  His Medical School studies were at the University of Illinois College of Medicine.  Subsequently, his professional training has been in the Chicago area at Advocate Christ Medical Center in Oak Lawn where he completed his residency training in Family Medicine.

Dr. Guzman remains a Chicago area resident, sharing his home with his wife and 2 daughters.  He firmly believes in following the same medical advice he gives to his patients and leads a healthy and active lifestyle.

Born into a medical family and a son of a General Surgeon, Dr. Guzman takes great pride in his work and is dedicated to providing competent and state of the art preventive Primary Care from newborn stage through geriatric age.

Dr. Juanita Valverde is a Family Medicine Physician who is board-certified.

Dr. Juanita Valverde has been in practice for 30+ years. She’s a graduate of the school of medicine at Universidad Atonoma De Ciudad Juárez,Mexico. Her formal training in Family

Medicine took place a Cook County hospital in Chicago Illinois.   Dr. Valverde has consistently taken an interest in the underserved community throughout the Greater Chicagoland Area. During her years in medical practice she has consistently taken an interest in the geriatric and adolescent population. She is well-versed in taking care of patients with chronic disease and preventive health.During her 30+ years in family medicine Dr. Valverde  has taken care of a diverse population which include the Latino , African-American,Polish and Arabic populations. She is well-versed in English and in Spanish and strives to ensure that her patients are taken care of in such a way that they receive high quality healthcare using the model of the medical home.

Ferdinand Catotal is a licensed Family Nurse Practitioner with Full Practice Authority specializing in Family Medicine in the state of Illinois and Wisconsin. Graduating with a Bachelor’s degree in Nursing Science from Olivet Nazarene University, he has served as a Registered Nurse and Advanced Cardiovascular Life Support Provider for the past 15 years before finishing his Master’s Degree in Family Nurse Practitioner Practice from Governors State University.

Fluent in English and Tagalog, and proficient in Spanish, Ferdinand continues to serve the Elgin and surrounding areas as a Nurse Practitioner at Elgin Surgicare and is a member of the American Association of Nurse Practitioners.

As a Nurse Practitioner, Ferdinand is committed to providing evidenced-based, holistic, patient-centered care in collaboration with partners through a shared decision making process to foster the best solutions to achieve healthy and fulfilling lives.
